Pre-completed FSQ Member Affiliation Forms: For those who were registered in 2009 and were Chelsea residents in 2009, pre-completed FSQ Member Affiliation Forms will be available at the Chelsea Town Hall and on registration night Tuesday March 16, 2010. Just make any needed corrections by hand and sign in blue ink.
