Pre-completed FSQ Member Affiliation Forms: For those who were registered with Chelsea in 2011 we will e-mail pre-completed FSQ forms in February but they will not otherwise be available at the March Registration Night or at the Chelsea Town Hall. Just make any needed corrections by hand and sign in blue ink. Blank FSQ Member Affiliation Forms will always be available.
